Devereux Advanced Behavioral Health Care Coordinator in Scottsdale, Arizona

Description

Position Details:

Pay Range: $19.00-$20.90/hr. (depending on education/experience)

Hiring Bonus: $1,000 (external applicants only)

Schedule: Monday-Friday 8am-5pm

Located on a beautiful 10-acre campus in the heart of Scottsdale, Devereux Arizona’s 24-hour RTC and Therapeutic School, has the capacity to serve 48 children. The RTC is a safe and supportive environment that provides youth, from the ages of 5 to 17, who are experiencing emotional, behavioral or substance abuse challenges. We also provide an on-site accredited K-12 therapeutic school, which offers an academic curriculum while students receive intensive behavioral and emotional support.

As a Care Coordinator, you will work cooperatively with clinical, operations, medical, and administrative staff on the overall coordination of individual care. This position participates in family engagement/outreach, admission/intake processes, discharge planning, treatment planning, and documentation of all care coordination activities accordingly.

Qualifications

  • HS Diploma or GED required; Bachelor’s degree in Human Services, psychology, social work or related field preferred

  • Four years’ experience with high school diploma/GED; OR one year experience with Bachelor’s degree

  • Experience in mental health/social services field required

  • Must demonstrate excellent verbal and written communication skills

  • Ability to handle multiple tasks and meet deadlines with minimum supervision; self-motivated individual

  • General knowledge of clinical best practices for working with adolescent populations and their families/resources

  • General knowledge of mental health, funding and related services within Arizona and other states served

  • General knowledge of standard office computer applications

OTHER:

  • Must be 21 years of age or older

  • Must be available to attend 2 weeks of required orientation training (paid, Mon-Fri 8:30am-5pm) plus additional 2-3 weeks on the job training

  • Valid Arizona Driver's License & pass DMV 39-month history check

  • Current AZ Fingerprint Clearance Card with no restrictions (or eligibility to obtain with no restrictions)

  • Pre-employment Physical and Job Demand Assessment (if applicable)

  • Drug test within 24hrs of offer w/ negative results

  • Devereux requires all individuals interviewing for a position involving contact with children to complete The Diana Screen, a screening that assesses a candidate’s understanding of appropriate boundaries between adults and children
