HYPE stands for Helping Youth Participate and Excel. As a Transition Facilitator, you will provide mentoring and independent living skills and training on a one to one basis with clients in the HYPE program. You will also provide family support to assist the clients to engage their families in a healthy and positive manner. The Transition Facilitator will offer guidance and coaching for clients to help them navigate the transition to adulthood.

High School Diploma or GED; Bachelor’s degree preferred
Behavioral Health experience: four years’ with high school diploma; one year with a Bachelor’s degree
Proficient with MS Office products, email, internet, and basic database navigation.
Must be available to attend 1 week of required training (paid, Mon-Fri 8:30am-5pm).
Valid Arizona Driver's License & pass DMV 39-month history check
Current AZ Fingerprint Clearance Card with no restrictions (or eligibility to obtain with no restrictions)
Pre-employment Physical and Job Demand Assessment (if applicable)
Drug test within 24hrs of offer w/ negative results.
