The Social Services Coordinator reports directly to the Program Director. This position is a remote position, but does require travel to the Treatment Homes at least once a week and to the office as needed/required.

What Devereux Looks for in YOU:

  • Responsible for acting as the program liaison between the treatment parents and the program administration including but not limited to parent training, consultation and implementing the program model,

  • Coordinating admissions, completion of admission documentation, facilitating meet and greets, and the coordination of discharge planning.

  • Monitoring and follow up on regulatory compliance with i.e. medical /dental appointments, psychiatric appointments, linkage to community resources, etc.

  • Coordinating and document the collateral services including but not limited to treatment plan meetings, attendance at community meetings, school meetings or other meetings/collaterals where assigned.

  • Act as the program liaison with external customers such as DYFS, CMO/YCM and legal families.

  • Timely collection and reporting of monthly forms, reports and data as required by the agency and program philosophy.

Qualifications

The Ideal Candidate Will Have:

  • Bachelor's degree in Social Work or related discipline.

  • One year of Case Management or Social Work experience.

  • Excellent verbal and written communication skills.

  • High level of initiative, accountability and professionalism.

  • Exceptional organizational and problem solving skills.

  • Ability to multi task and be deadline driven.
