Devereux Advanced Behavioral Health Arizona
is seeking a talented individual to join our team as a
Care Coordination Specialist . This position will work with our Outpatient program, supporting our Respite program with Case Management duties. We are seeking candidates who can work collaboratively with clinical, direct care, administrative staff and Child and Family Team (CFT) members, to provide the necessary support to coordinate treatment, case management and the maintenance of assigned client files. Responding to case inquiries, assisting with review of referrals, and provides assistance with service planning and review.**
This position will have a Monday - Friday 8:30am-5:00pm schedule.
What would a day in this position look like?
You would be responsible for keeping treatment anc CFT informed of all cases via email, meetings, and telephone. Keeping records/files up to date is vital for the success of this role. You will respond to care management inquiries from clinical staff or referring agency and any parent/case manager/funder requests for information.
Devereux is seeking a Care Coordination Specialist who can demonstrate knowledge and skills in case management, and in providing appropriate care, treatment, and services for the behavioral health needs of the children and asolescents we serve. Having experience or familiarity with common mental disorders, as well as understanding the principles of human growth and development over the lifespan.

Education : Bachelors degree in Human Services or related field preferred.
High school diploma or GED required.**
Experience : One year of work experience in mental health delivery system. Knowledge and background in assessment and treatment planning development.**
Ability to make decisions and meet deadlines
Exceptional communication skills, being able to interact with diverse cultures with clients and staff
Must be computer literate
Must possess Level One AZ Fingerprint Clearance Card
Must pass 39-month MVR Check
