is seeking a qualified

full - time Moderate Needs Case Manager

to be a part of our Outpatient team in Phoenix! Devereux offers comprehensive outpatient counseling services for individuals, families, and groups in the office, school & or home setting. Specialties include the treatment of ADHD, Conduct Disorders, Mood Disorders, Physical Abuse, Sexual Abuse and Anxiety Disorders. Counseling services may be supplemented by psychiatric intervention and medication management when indicated.**

Schedule is Monday - Friday 8:00am - 5:00pm, this can vary on some days based on the needs of the program and clients. Flexibility to alter hours in this role can and will be needed based on client needs. This position is based solely out of our Outpatient office in Phoenix, no field work is required for this role.

Job Duties:

  • Our Moderate Needs Case Manager facilitates child and family team meetings and has the primary responsibility for completion of documentation from all providers

  • The lead for moderately complex cases to coordinator behavioral health services, medical care, education, placement, finances, and socialization

  • Serves as liaison for the team, coordinating the services the youth receives

  • Develop master treatment plan for client, family, and all necessary behavioral health providers

Requirements

  • Bachelor’s degree in psychology, sociology, Human Service, Social Work or related field. Master’s degree preferred. OR High School Diploma/GED and six (6) years of direct experience.

  • One (1) year of experience in high needs case management required

  • Bilingual in English and Spanish preferred

  • Valid Arizona Driver’s License

  • Current unrestricted AZ Level One Fingerprint Clearance Card
