Devereux PA Adult Services provides care and residential living to nearly 400 adults with intellectual and developmental challenges in homes, apartments and a long-term care community in southeastern Pennsylvania. (Chester, Montgomery & Delaware Counties)

We are currently seeking a Full Time
to work collaboratively with ex/internal stakeholders including: agencies, families, Individuals, and Devereux operational departments, in the coordination and implementation of an Individual's Support Plan (ISP). Ensures that: all supports and services identified in the ISP are provided, and are in compliance with applicable service definitions and state and federal funding requirements and regulations; the support plan is relevant and comprehensive; obtaining complete & accurate documentation which evidences the delivery of services
The position will have a caseload of NO MORE than 30 individuals. Coordination and Completion of the comprehensive assessment. Development of the ISP, ISP Annual Update and all ISP revisions. Attendance to all ISP, ISP Annual Update and ISP revision meetingsThe location of this position is in Berwyn, PA. Hours are 8:30am-5pm M-F.
Hourly rates for this position is $12.74-$14.52.
Service Delivery :
Demonstrates the skills, knowledge, and ability to coordinate and implement individualized program services. Designs services to meet specific therapeutic needs based on individualized program plans, goals, and objectives.
Monitors and coordinates the services in the ISP and monitors service authorization process. Strategically addresses unrecognized or underutilized services which would enhance the Individual's quality of care and life. Schedules and facilitates ISP meetings. Instructs and monitors Program Supervisors on goal development and documentation and the implementation of goals and behavior plans. Assist with providing in-house training, as needed. Prepares a “Goals at a Glance” guide for staff which lists current goals for each Individual. Works with the Supervisor to develop, monitor, and document implementation of program activities, goal implementation, and other supported services. Maintains effective and efficient communication with families and agencies. Serves as a liaison for the Individual, the Interdisciplinary Team, parents, and other agencies via telephone contacts, written documentation, and on-site visits. Maintains monthly contact with families or appropriate individual contact; is responsible for inclusion of vocational program information in the ISPEnsures that all services and tracking of services are in compliance with state and federal funding &/or regulatory requirements.
Delivers services in such a manner that they attend to quality issues (data collection, trend analysis, outcomes, corrective actions), the implementation of Best Practices, innovative methods of treatment and treatment delivery, and excellent customer care.**
Documentation & Data Management:
Develops and maintains a database for goal creation. Provides monthly synopsis based on each persons progress and status, to include information about units-allocation, remaining units, and service authorization dates - taking appropriate action to address either over/under utilization. Compiles any additional data and statistical reporting as requested. Collaborates across discipines to approrpiately communicate and address: admissions, discharges, transfers, deaths, specialized services, and therapeutic &/or medical leaves.
Ensures that all required documentation is completed and submitted to County/State Offices for continuation of eligibility of benefits. Assists in the restoration of eligibility.
Has working knowledge of MA and Insurance eligibility and regulations and assists Administration and Nursing with insurance eligibility and coordination of benefits and periodically verifies the accuracy of insurance information.
Responsible for ensuring that services that are or will be provided by Devereux are included in the appropriate narrative and Service Details/Service Summary Sections of the ISP. Ensures that Devereux has authorization from external funder for services and are in Devereux’s Client Information System and obtains written authorizations for any uncovered services. Schedules and coordinates resident reports (IHP and Quaterly reviews). Maintains documentation in accordance with established procedures, using the Devereux approved format.**
DPW Regulations/Guidelines as required by ODP- Reviewing the ISP, ISP Annual Update and all ISP revisions. Reporting content discrepancies to the S.C. as applicable and plan team members.
Implementing the plan as written.
Supervising, monitoring and evaluating services. Reviewing, signing and dating monthly documentation of individuals’ participation and progress toward outcomes. (The IDT note or monthly progress note serves this purpose).
Reporting a change relating to individual’s needs to the SC as applicable. Reviewing the ISP with individual. Documenting review of plan (agree/disagree form). Providing documentation of plan review to SC as applicable. Informing plan team members of the option to decline the ISP review (agree/disagree form). Documenting review of plan (Quarterlies and Revisions). Recommending a revision to a service or outcome in the ISP. Coordination of services provided to the individual.Coordination of training of DSP’s in the content of Health and Safety needs relevant to each individual (Health and Safety is included in the assessment and ISP). Developing and implementing provider services.**

Bachelor's Degree from an accredited program in an area of human services, which includes sociology, psychology, social work etc. AND two (2) years experience in the Human Services field, working with adults with intellectual/developmental disabilities, mental health diagnoses, and/or TBI.
Requires knowledge of regulatory requirements and an ability to adhere to established regulations, excellent customer service skills, as well as written and verbal communication skills, organizational skills, and problem-solving skills.
Must be able to adapt to rapidly changing demands of the position, customer needs, and the organization.
Must be proficient in Word Processing and Database management.
Must possess and maintain valid driver's license, and be at least 21 years of age
